Registering as a New Patient

In order to register with the Practice you will be asked to complete a form with details of your previous doctor and previous address. A receptionist will help you to do this. You will be given an appointment with our Practice NurseĀ or with one of the doctors, who will carry out a New Patient Medical, which includes details of previous illnesses, medications, blood pressure and urine tests along with general lifestyle advice. The doctor retains the right to refuse to accept a new patient, and in this case will write to you stating his or her reasons.

When attending the practice to register, please bring photographic ID to confirm your entitlement to register for NHS GP services. You can download the GP Registration form (here) and our New Patient Information Form (here).

Please note we require you to complete both of the above forms to register.

Change of Personal Details

Any changes of name, address or telephone number should be notified in writing to the practice as soon as possible. Alternatively, forms are available at reception which you may complete or can be downloaded here.
